CHOICES Program
Professional Care at Home or in a Nursing Home

TennCare CHOICES in Long-Term Care is TennCare's program for long-term care services. These are also called long-term services and supports (LTSS). They include help doing everyday activities that you may no longer be able to do for yourself as you grow older, or if you have a disability—things like bathing, dressing, getting around your home, preparing meals, or doing household chores.

LTSS also includes care in a nursing home, in your home, or in the community that may keep you from having to go to a nursing home for as long as possible. These are called Home- and Community-Based Services or HCBS.

WHAT IS CHOICES?
Tennessee's CHOICES program includes nursing facility services and home and community-based services (HCBS) for adults 21 years of age and older with a physical disability and seniors (age 65 and older).

WHAT SERVICES ARE COVERED BY CHOICES?
Services range from personal care to home-delivered meals to respite and adult day care, and are based on the individual needs of the CHOICES consumer. CLICK HERE to learn more.

WHO WILL MANAGE THE SERVICES I RECEIVE THROUGH CHOICES?
Your TennCare health plan or Managed Care Organization (MCO) will be responsible for managing all of your physical health, mental health and long-term care needs, and the services that you receive to address these needs. CLICK HERE to learn more.

WHAT IS CONSUMER DIRECTION?
Consumer Direction is a way of getting some of the kinds of home care you need. It offers more choice and control over who provides your home care and how your care is given. CLICK HERE to learn more.

WHAT IS ESTATE RECOVERY?
If a person receives Medicaid long-term services and supports, the State of Tennessee must ask for money back from that person’s estate after he or she dies to help pay for certain kinds of care he or she received. The money that is collected goes back to TennCare to help pay for long-term services and supports for others. CLICK HERE to learn more.

HOW DO I APPLY?
If you have TennCare and are interested in the CHOICES Long-Term Services and Supports program, you can call your TennCare health plan (MCO). The number is on your TennCare card. If you don't have TennCare, and are interested in CHOICES, contact the First Tennessee Area Agency on Aging and Disability (FTAAAD) at 1-866-836-6678 to see if you might be eligible.
