Choices

TennCare's Choices Program is intended to provide a community based, cost effective alternative to institutional nursing facility care for eligible adults.  An array of services are available for those who meet program requirements.

If you have TennCare, you can call your TennCare health plan (MCO) to apply. The number is on your TennCare card.

If you don’t have TennCare, Contact us toll free at 1-866-836-6678. If you don’t qualify for Medicaid, we may be able to inform you about other programs that may help.
